About The Speakers
Jackee Namila, BCPA
Jackee C. Namwila, BCPA, is a healthcare consultant. Through her two companies — Namsi Healthcare Consulting and Patient Advocates, Inc. and Empire Healthcare Management, LLC — she has consulted with the Attorney General's office as an Independent Monitor; she has also consulted with Article 28s, IDTFs, Home Care Agencies, Federally Qualified Health Centers (FQHCs) and OBS/Ambulatory Surgery Centers. Additionally, her firm continues to play a major role as an MSO to a mental health clinic located in Harlem, NY (Tele mental health).
Ms. Namwila recently became a Board-Certified Patient Advocate (BCPA); she is also active on the Medical Advisory Council to Mildred Elley College and ASA College.
Previously, she was Director of Managed Care, Director of the Physician Hospital Organization (PHO), and Executive Director of the Sleep Lab at the New York Eye and Ear Infirmary.

David is a Board Certified Patient Advocate and the director of Engage Caring Solutions. He is a passionate leader committed to improving the lives of residents in nursing homes. His unique background as a highly regarded licensed nursing home administrator and knowledgeable regulator in the New York metropolitan area make him one of the most qualified experts in his field. Throughout his career, David has earned his reputation as a stickler for detail and for holding surveyors, staff and owners accountable for delivering resident care and service that meets or exceeds residents’ needs on a daily basis.

Preventing Unsafe Discharges
The hospital discharge process can be stressful and frustrating to patients and their families alike. When discharges are poorly planned, families are coerced to make critical care decisionsat the last minute, immense pressure on families to make critical decisions without sufficient time or information for choices to be informed. This ACES presentation will identify the key role and tenants of effective patient/family advocacy when patients are discharged home or to a Skilled Nursing Facility/Rehab Center. Each scenario is fraught with challenges that must be understood and addressed proactively. We hope to provide a clear understanding of the enormous pressures that drive each provider’s incentives and systems (Hospitals,SNF/Rehab Ctrs and Home Health Agencies) and provide concrete actions advocates can taketo stay informed and actively involved in the Discharge Planning process to ensure a safe discharge.
